About Damm Pharmacy

Damm Pharmacy is a locally-owned community pharmacy with four locations dedicated to providing personalized service to patients at an affordable price. They offer a wide variety of services including conventional prescription filling, compounding, medication synchronization and much more. Their expertly trained pharmacists and friendly staff pride themselves on making sure each patient's pharmacy experience exceeds expectations.

Damm Pharmacy will be working with a local provider clinic in Derby, KS. This clinic has two physicians as well as several non-physician practitioners. The targeted services are Annual Wellness Visits, Chronic Care Management and Remote Physiologic Monitoring.

Fellowship Site Coordinator

Renee and Dared Price own six pharmacies in South Central Kansas. They are very involved in pharmacy organizations; primarily CPESN KS (Renee serves as the Managing Network Facilitator), KPhA (Dared currently serves as President) and NCPA (Dared is a Board Member). In addition to organizational involvement, they focus on bringing new opportunities to their pharmacies, Avant Fellowship and physician collaboration being the current top priority! The Prices live in Winfield, KS with their three boys and spend their free time watching athletic events.

Chronic Care Management

Management for Medicare patients with 2 or more chronic conditions

Remote Physiologic Monitoring

Management of digitally collected physiologic data
