Our organization has been offering pharmacist-led collaborative clinical services since 2014. This collaboration started with clinics in Charlotte, North Carolina, and has since expanded to areas of South Carolina with partnerships ranging from independent primary care providers to Federally Qualified Health Centers (FQHCs) and Accountable Care Organizations (ACOs). Avant Pharmacy and Wellness (formerly known as Rx Clinic Pharmacy) is fellow to lead the clinical service programs with our partnered FQHC look-alike. The fellow will serve as the primary clinical pharmacist at these clinics.

This will be the first year we are offering a community-focused position within our fellowship program. Our community fellow will drive patient outcomes through community clinical programs, such as MTM, contracts with private payers, and wellness program initiatives. There will also be a staffing component integrated into the community position so the fellow gains a solid knowledge of the impact that can be made through identifying drug therapy problems at this point in care.

Join our innovative team as a pharmacist fellow, working alongside our Director of Health and Wellness to expand patient-centered services. You’ll be involved in implementing a dementia care model to support patient independence at home, collaborating with home care agencies and assisted living facilities. Additionally, you'll help lead comprehensive weight management programs, incorporating lifestyle, behavioral, and medical approaches to support patient success. Your role will extend to assisting with medical infusions and IV hydration by coordinating patient care, monitoring medications, and ensuring adherence. You’ll also support hypertension and diabetes therapy management while contributing to the expansion of pharmacy services and fostering interprofessional collaborations to enhance patient care.

Brianna Johnson, PharmD, has 8 years of experience specializing in geriatric and collaborative care with independent physicians. A graduate of Wingate University School of Pharmacy, she excels in initiating and coordinating collaborative programs. Brianna has presented on non-face-to-face programs for multiple pharmacy organizations, created and taught a certificate program with APhA, and co-facilitated a fellowship with the Avant Institute. Outside of work, she enjoys spending time with her family, camping, reading, and cooking.

Amy Kern completed her Doctorate of Pharmacy degree from North Dakota State University and immediately moved to Charlotte, desperately searching for warmer weather. Over her 20+ years in pharmacy, she has experience working for both chain and independent pharmacies, which allowed her the opportunity to explore specialized areas in pharmacy, including compounding hormone replacement therapies, managing care coordination for long-acting injectables, coordinating COVID-19 vaccination clinics during the early, challenging days of vaccine availability, managing formularies when 340B manufacturers added restrictions to their medications, and coordinating processes for providers to Buy & Bill HIV treatment and prevention injectable medications. Working on the 340B program has been so rewarding; She is passionate about helping patients access cost-saving alternatives without compromising the quality of their treatment, ensuring better medication adherence and overall health outcomes.

Jack Novansy graduated from East Tennessee State University with a Doctorate of Pharmacy and Masters in Business Administration. He serves as the Director of Health and Wellness at Avant Pharmacy & Wellness Center. Dr. Novansy is passionate about combining his expertise in exercise science and pharmaceuticals to optimize health. He believes in the importance of living active and healthy lifestyles that are both practical and rewarding. As a pharmacist with Avant, he takes great joy in helping patients challenge their current conditions and achieve their best health.
