Started in 2000 by a group of committed volunteers, Charlotte Community Health Clinic offers high-quality medical, dental, and behavioral health services for kids and adults.

They are looking forward to getting Annual Wellness Visits and Chronic Care Management started for their Medicare population. 

What are the primary pharmacist-led services?

Annual wellness visits (AWVs)

Annual comprehensive visit for Medicare patients

Chronic care management (CCM)

Management for Medicare patients with 2 or more chronic conditions

Remote physiologic monitoring (RPM)

Management of digitally collected physiologic data

What skillsets are needed to be successful?
