Faith Medical Center, led by Dr. Joel Onafowokan, is a patient‑centered multispecialty clinic committed to delivering comprehensive care in a supportive, family‑friendly environment. Our clinical focus includes primary care, pulmonology, cardiology, mental health, and substance use disorder treatment. This creates the perfect setting for pharmacists who want to practice at the top of their license and grow in a rich clinical environment.

Our goal as a first-year fellowship site is to create an environment where pharmacists are integral to our care model. As a new graduate, you will step into a practice that values your clinical training, encourages independent thinking, and provides the mentorship needed to build confidence in direct patient care. The fellows will initiate Medicare Annual Wellness Visits and build a Chronic Care Management and Remote Patient Monitoring program to meet value-based quality metric goals and grow the practice.
