Founded in 1979, Medlink Georgia is a Federally Qualified Community Health Center serving 69,013 patients through 34 locations in Northeast Georgia. In addition to primary care, we also offer Pharmacy Services with 6 in-house pharmacies and over 20 contract pharmacies. In addition, we offer pediatrics, behavioral health, diabetes and nutrition education, women’s health, medication-assisted treatment, and general dentistry. As a Patient Centered Medical Home, we have begun to have a large emphasis on Population Health Management and look forward to growing these services with Clinical Pharmacists.

Graduating from UGA Pharmacy School in 2000, Dennis spent 15 years as a pharmacy manager at Walmart. For the past 9 years, he has been with Medlink Georgia, where he has overseen growth from 1 to 6 (soon to be 8!) pharmacies as Pharmacy Director. He is the proud parent of 5 children and also an ordained minister who had the honor of marrying two of his pharmacy staff members last year.

Tiffany O'Donnell PharmD is a graduate of The Mercer University Southern School of Pharmacy. She has practiced in community pharmacy for nineteen years serving in chain pharmacy, independent pharmacy, insurance outcomes, and currently 340B FHQC pharmacy with Medlink. She is passionate about patient care and making sure patients not only understand their medications, but also are adherent and have access to the tools and resources to have the best outcomes with management of their disease states. She is committed to helping students develop strong clinical skills and professional judgement while educating the communities in which they serve.  

What are the primary pharmacist-led services?

Chronic Care Management (CCM)

Management for Medicare patients with 2 or more chronic conditions

Remote Physiologic Monitoring (RPM)

Management of digitally collected physiologic data

Quality Care Gap Initiatives

Development of a workflow to address a risk-stratified population and significant gaps in care

Pharmacy-driven initiatives

Additional opportunities aligned with Population Health Management including addressing prior authorizations and medication adherence
