Founded in 1979, Medlink Georgia is a Federally Qualified Community Health Center serving 69,013 patients through 34 locations in Northeast Georgia. In addition to primary care, we also offer Pharmacy Services with 6 in-house pharmacies and over 20 contract pharmacies. In addition, we offer pediatrics, behavioral health, diabetes and nutrition education, women’s health, medication-assisted treatment, and general dentistry. As a Patient Centered Medical Home, we have begun to have a large emphasis on Population Health Management and look forward to growing these services with Clinical Pharmacists.

What are the primary pharmacist-led services?

Pharmacy-driven Initiatives

Primary opportunities aligned with Population Health Management including care coordination and  addressing prior authorizations. 

Quality Care Gap Workflows

Development of a workflow to address a risk-stratified population and significant gaps in care to meet key quality metric goals such as medication adherence.

Exploration of Collaborative Clinical Services

Additional opportunities may include collaborative practice agreements, Medicare Annual Wellness Visits, and more.
