What is the focus for the fellowship?
Annual Wellness Visit
Annual comprehensive visit for Medicare patients
Chronic Care Management
Management for Medicare patients with 2 or more chronic conditions
Transitional Care Management
Management of patients who have recently transitioned from the inpatient to outpatient setting

Entrepreneurial approach to building programs
