Population Health and Practice Development Fellowship

Application Materials

The candidate must have a Doctor of Pharmacy degree and must be eligible for licensure in the state of their respective site. Applications must be received by April 22, 2024, for round 2 interviews.

Materials must include:
  • Letter of intent: Describe your reasons for pursuing this fellowship and how completion of this program will help achieve your career goals (please include a separate letter of intent for each site you designate on your application).
  • Curriculum Vitae
  • Essay: Include a response to each of the following questions (limit three pages total)
    • Articulate your vision for innovative pharmacy practice, how to accomplish this, and the positives and challenges of innovative practice
    • Describe your personal strengths and how you will utilize these strengths to advance the profession
  • Three references (The Avant Institute reaches out to references directly, so full reference letters are not needed in advance.)
  • Transcript (can be sent to info@avantinstitute.com)
