Our organization has been offering pharmacist-led collaborative clinical services since 2014. This collaboration started with clinics in Charlotte, North Carolina, and has since expanded to areas of South Carolina with partnerships ranging from independent primary care providers to Federally Qualified Health Centers (FQHCs) and Accountable Care Organizations (ACOs). Avant Pharmacy and Wellness (formerly known as Rx Clinic Pharmacy) has several key primary care partner clinics in the Gastonia area that are seeking a fellow to lead the clinical service programs. The fellow will serve as the primary clinical pharmacist at these clinics.
