Optimize care for patients through pharmacist-led MTM services. The goal of this training program is to increase the participation of pharmacists in the prevention and management of chronic disease, specifically through the use of Medication Therapy Management (MTM) services.

Medication Therapy Management (MTM) Services

  Module 1
  1. Articulate the value of the pharmacist in impacting the quadruple aim.
  2. List common barriers for implementation of MTM programs as well as proposed solutions for these barriers.
  3. Describe how Star Ratings are impacted by CMR completion rates.
  4. Describe how pharmacists bring value to MTM services.
  Module 2
  1. Explain the difference between a comprehensive medication review (CMR) and a targeted medication review (TMR).
  2. Describe the key features of the OutcomesMTM platform.
  3. List the four main models of the Part D Enhanced Medication Therapy Management Model.
  Module 3
  1. Describe strategies for optimizing workflow for MTM activities.
  2. List example MTM activities that can be led by support staff, such as pharmacy technicians or interns.
  3. Compare and contrast motivational interviewing and integrative health coaching as well as how they can be utilized for MTM services.
  4. Distinguish between the process of working up a CMR and a TMR.
  5. Calculate the cost savings of delegation of MTM activities from a pharmacist to another team member.
  6. Compare and contrast MTM services and chronic care management (CCM) services.
  Implementation
  1. Explain common interventions and scenarios encountered through MTM activities.
  2. Distinguish between the process of working up a CMR and a TMR.
  3. Calculate the cost savings of delegation of MTM activities from a pharmacist to another team member.
  4. Compare and contrast MTM services and chronic care management (CCM) services.
