Population Health and Practice Development Fellowship

Application Materials

The candidate must have a Doctor of Pharmacy degree and must be eligible for licensure in the state of their respective site. Applications for the remaining two fellowship positions with Damm Pharmacy in Derby, KS, and Moose Pharmacy in Salisbury, NC, will be accepted on a rolling basis until May 31, 2024.

Materials must include:
  • Letter of intent: Describe your reasons for pursuing this fellowship and how completion of this program will help achieve your career goals (please include a separate letter of intent for each site you designate on your application).
  • Curriculum Vitae
  • Essay: Include a response to each of the following questions (limit three pages total)
    • Articulate your vision for innovative pharmacy practice, how to accomplish this, and the positives and challenges of innovative practice
    • Describe your personal strengths and how you will utilize these strengths to advance the profession
  • Three references (The Avant Institute reaches out to references directly, so full reference letters are not needed in advance.)
  • Transcript (can be sent to info@avantinstitute.com)
